The GOOD NEWS is that there is FREE support for you to make these changes successfully!
Health Trainers are experts in health behaviour change and will work with you to set realistic goals as part of a personal health plan to achieve the long term results you want. They can also signpost people to other health and care services and are also able to issue 'social prescriptions' These are referrals to other groups and organisations that can help the individual.
We now have a Health Trainer available every Tuesday, based alternate weeks at Ambrose or Tollgate.
You can be referred by your GP or you can Self Refer.
