Text Messaging Service
As form 1st April 2018, the laws regarding General Data Protection are changing. This means that we will no longer be able to send you Text message reminders to your Mobile regarding appointments and results unless you have signed a consent form.
In order for us to continue providing this useful service to you, our patients, we urge you to call into the surgery at Ambrose Avenue or Tollgate to sign out consent form.
Alternatively, just head over to the Text Message Reminders page on the left to download a form and send it to us.
It only takes a minute and means we will be able to continue keeping you up to date!
We apologise that this can not be completed online, we do need a formal signed copy of your signature before it will be accepted by the General Data Protection Regulations!

To access this system you first need to obtain Log In details from one of our Reception Team at either Ambrose or Tollgate, you will be asked for PROOF OF ID before we can give you the login details. Once you have your log in details come back to this website and click on the icon.
Please note this service will be for ROUTINE APPOINTMENTS ONLY. This service will NOT allow booking of DOUBLE APPOINTMENTS - if you need a double appointment please speak to a member of our reception team.

Our dedicated team are here to treat those minor ailments that occur as well as providing specialist management of long-term conditions and clinics covering a wide range of healthcare issues. The technology also means you can now do a lot of things from the comfort of your home such as order a repeat prescription or cancel an appointment.
Access - The practice premises at both Ambrose Avenue and Tollgate Health Centre have suitable access for disabled patients. There is one designated parking space for disabled patients at Ambrose Avenue and four at Tollgate Health Centre.
